LVN/LPN Case Manager, Telehealth; BiLingual (HINDI/English); PT; tele-commute
NOTE: Language and license requirements are absolute). We are seeking a part-time Nurse Case Manager (CM) fluent in Hindi and English, licensed and in good standing in any Nursing Compact State. You will work from home on behalf of our physician partners, helping patients successfully manage their behaviors, understanding and attitudes resulting from multiple chronic conditions. You will work with our clinical team to develop, implement, and evaluate individualized patient care plans. You will instruct, coach and mentor patients; and advocate patient welfare. CMs serve as a liaison between patients, families, and their healthcare providers.
CMs work from home on a flexible schedule. Must have broadband internet service, willingness to work in videoconference, and able to quickly learn our cloud-based EMR and patient engagement tools.
Bi-lingual Nurse Case Manager Job Duties
The primary role of the CM is to assess barriers to patient self care and management at home; create solutions and gather resources to assist the patient to overcome those barriers; and in an ongoing process of assessment and encouragement, work to achieve healthier outcomes for the patient at risk for clinical deterioration due to multiple comorbid conditions.
Bi-lingual Nurse Case Manager Requirements
Nurse case manager must be an LVN or LPN in good standing with degree from an accredited nursing program. Must be currently licensed in a Nursing Compact State, fluent in both Spanish and English. Additional useful skills/coursework includes understanding of Quality Measures, Legal and Ethical Issues in Case Management, Case Management Process, and Designing Case Management Models.
We seek nurses who embrace patient care, love working with patient and their family over a longer term, and can manage our telehealth technology. Our nurses are patient and empathetic, understand motivational interviewing for behavior change as well as managing medical factors.
CM must have an appropriate space and conditions at home for seeing patients using videoconferencing and Internet-based electronic medical records.
4-8 hours per week to start; could lead to full-time. Hours are flexible, including evenings.
Commensurate with experience, training, and national averages. Position is available immediately.
